The Aging Life Care Management of New Mexico will help you articulate your individual goals, discuss various options to achieve your goals, help navigate health systems and ensure you or your loved one receive the best care and resources available. Our process involves two important phases, Assessment & Care Planning and Care Coordination & Monitoring.

Assessment & Care Planning

Aging and disability bring with them unique challenges and frail elderly aspects that must be promptly and properly addressed to ensure a continued high quality of life and dignity. The Aging Life Care Management of New Mexico provides critical services to help assess a frail individual’s situation and create an initial care plan. Initial care management services may include a professional geriatric consultation and the development of a care plan.

A licensed professional who is specially trained in meeting the challenges of aging will spend an hour or two with your loved one in their home or at the offices of The Aging Life Care Management of New Mexico to conduct a professional geriatric consultation. Your loved one’s needs will be discussed at length, and you’ll learn about the options, local services, and resources that are most appropriate for your loved one’s individual situation. The Aging Life Care Management of New Mexico will verbally outline our suggestions for a successful resolution.

Care Planning is a two-hour evaluation with an experienced New Mexico elder care professional. Family members and caregivers are welcomed at this in-person interview which will cover a broad range of topics critical to frail elderly health aspects, including, but not limited to:

  • Everyday Activities
  • Nutritional Status
  • Safety
  • Cognition
  • Mental Status
  • Finances
  • Insurance

Your loved one has specific needs, health issues, and concerns. In order to appropriately plan for their future, The Aging Life Care Management must accurately assess any issues your loved one or you as a care provider or family member are facing. After the assessment, you will receive a written report with detailed findings, recommendations, and an initial care plan that addresses each issue. You’ll receive information about local resources and elder care services. The program can be completed within 14 days plus time for mail delivery.

The Aging Life Care Management care manager can also meet with the senior and family members to review and discuss the written care plan.

In spite of your initial care plan, frail elderly health aspects and living situations may change over time. The initial care plan is a foundation that may be altered or built upon as needed. Our team of professional consultants and elder care mangers will take a holistic view of your aging loved one’s situation and apply any necessary changes to ensure your loved one’s health and well-being.

Care Coordination & Monitoring

One of the most difficult aspects of care giving may be putting an elder care and health plan into action. Many tasks may feel overwhelming or time consuming. Care givers may not have the time or ability to research everything required by the recommended elder care and health plan.

The Aging Life Care Management of New Mexico’s Care Coordinating Program and Care Monitoring Program are effective solutions for implementing a care plan and making sure it is meeting the needs of your elderly loved one.

We understand that the toughest part of elder care can be putting a Care Plan into action once the key issues are determined. Oftentimes, the tasks required to implement the Care Plan are enormous and time consuming and many care givers just do not have the time or capacity to research and arrange all of the services to best meet their elder’s needs.

Our Care Coordination Program was designed to take the weight of responsibility off of your shoulders. The program is the most convenient solution to searching and arranging services and products for your elder based on their specific needs and preferences.

When you authorize a Aging Life Care Management care manager to handle the care coordination, we will do all of the research, arrange all of the services, and fill out all of the paperwork to accommodate your loved one’s needs. Moving an elder from their home to a long-term care facility is never easy. If such a need should arise, you can rest assured that we will enlist the help of people who specialize in moving seniors to ensure the transition is as smooth as possible.

Care Monitoring provides ongoing oversight of services provided. A licensed, professional practitioner from our team of skilled New Mexico Care Managers will notify you if a change in status occurs or if a change in plan is needed.

One of our professional Care Managers will regularly check in with your loved one at their home or care facility to ensure their needs are being met and that they are comfortable. Our Care Managers will also take the time to review the elder care health plan to ensure it is accurate and up-to-date and coordinate any necessary changes.
