Our Care Management Process
Aging and disability bring with them unique challenges and aspects of geriatric care management that must be promptly and properly addressed to ensure a continued high quality of life and dignity. The team at Aging Life provides elder care assistance services to help assess an individual’s situation and create an initial care plan.
Professional Geriatric Consultation
Our Aging Life care specialists will spend an hour or two with your loved one in their home or at our offices in Albuquerque or Santa Fe to conduct a professional geriatric consultation. Your loved one’s needs will be discussed at length, and you’ll learn about the options, local services, and resources that are most appropriate for your loved one’s individual situation. Aging Life will verbally outline our suggestions for a successful ongoing plan.
We will make a care plan based on their needs and then implement the care plan. We will begin by taking a look at:
Based on the initial consultation, we will then implement a plan, which may include care coordination and care monitoring. Care coordination may include arranging for caregivers or other services needed in the home, completing paperwork (medical equipment orders, ordering medical records, etc.), or even coordinating a client’s move from their home to a community (assisted living, memory unit or even just downsizing to a small independent living community). Care Monitoring may include:
One or more scheduled visits to the elder’s home or facility every month for a status evaluation
Regular email or telephone consultations with family to discuss any issues that arise
Coordination and interaction with outside service providers and facility staff
Monthly email updates to family members and other appropriate parties, such as attorneys, trust officers, and physicians
Accompanying elders to medical appointments and emergency room visits
24/7 access 365 days a year to our emergency on-call service (Additional hourly fees apply when service is provided, but you always have access to our guaranteed response whenever you need it.
